NeuroRehabilitative Services are offered, arranged, and coordinated by members of the Rehab Services Team with facility oversight and leadership provided by the Administrator. Every individual admitted is unique with varying medical and clinical needs, goals, and wants.

NeuroCentered Rehab & Supported Living (NCRSL) prides itself on preparing customized Goal Plans which are updated as each individual progresses or when changes are noted. Discharge Planning expectations are discussed on admission to NCRSL and the anticipated discharge options are included in each Goal Plan.

The Referral Process

Prior to admission of each individual referred, a comprehensive intake is completed followed by a medical record review and an assessment. This ensures clinical relevance and, if accepted, a customized Proposed Plan is developed addressing clinical needs and the goals, needs, and wishes of each individual.

Adhering to this process is most productive in the establishment and provision of therapeutic and meaningful services and activities which most often result in the most successful clinical and financial outcomes.

Supported Living Services

NeuroCentered Rehab & Supported Living offers three levels of therapeutic, purposeful, and meaningful support services and care.

1. Transitional Short-Term Supported Living
Ensures clinically relevant continuity of care while containing costs

This level of Supported Living offers unique services ideal for individuals who require a transitional period between hospitalization and acute rehab, post-acute rehab, home care services, and pending discharge to home from a high-cost rehab facility when home modifications are delayed. It is effective and therapeutic for participating individuals and highly effective at cost containment.

Some Circumstances:

Physical injuries with orders restricting physical activities and/or formal therapies preventing them from participating in an intensive or full therapy program
Multiple bodily injuries such as bilateral upper and/or lower fractures or impairments limiting function, safety and self-care
Injuries to the back and spine with restrictions
Individual with paraplegia who has completed intensive rehabilitation but cannot go home as modifications are delayed and/or discharge home is a safety risk

Individualized Services Under the Above Circumstances:

A specific level of close to visual supervision for safety
Assistance with daily activities and care including ambulation and transfers, bathing and dressing, hygiene
Moderate exercise as tolerated (depending on the injury, diagnosis, or physician order)
Basic environmental maintenance
2. Extended and Long-Term Supported Living
The optimal opportunity for individuals with neurologic injuries to enjoy a meaningful and purposeful life while maintaining critical long term financial resources

A lovely homelike environment is the ideal living opportunity for individuals who have tried and have been unable to achieve independence following multiple attempts in rehabilitation centers, residential facilities, group homes, post-acute programs, supervised apartments, and/or when residing at home with family or home health services providing care and supervision.

Previous attempts in rehabilitation centers unsuccessful
Unable to achieve independence in residential facilities or group homes
Post-acute programs and supervised apartments unsuccessful
No longer able to live at home with family or home health services
Customized services all-inclusive as outlined in each Plan
3. Short Term Respite Supported Living
Family caregiver support

This level of Supported Living offers respite services in cases where individuals living with and/or cared for by family members or other caregivers can admit on a short-term basis and can choose to participate in functional and meaningful daily activities available.
